THE APPLICATION PACK ARE IN TWO GROUPS
FORMS 01.02.02A,03 ARE FOR THE SCHOOLS OR ORGANISATION TO COMPLETE IF NOT COMPLETED LEARNER WILL NOT BE ABLE TO START
FORMS 04,05,06,07,08,09,10 & 12 ARE FOR THE PARENT OR GUARDIAN TO COMPLETE BEFORE THE LEARNER CAN START
For Health and Safety and insurance cover it will be necessary to have all requested forms in our possession before the Student can take part in the courses.
